In order to be considered for qualification as a charity to receive donated equipment, please submit the following information for review:

1. Copy of mission statement

2. 501 Non-Profit Status Letter

3. Tax ID Number

Once this information is submitted to our office, it will be reviewed for registration. Please submit all information to:
